Attending Physician
Certifies the patient has a terminal illness and remains the patient’s primary physician and works as a member of the hospice team
Skilled Nurses RN’s & LVN’s
Evaluate’ s patient’s condition during regular scheduled visits
The registered nurse coordinates the plan of care and teaches the family proper patient care techniques; the use of medication and management
Social Worker
Assess social needs of the patient’s, family
Provides psychological and Bereavement counseling
Chaplain
Provides spiritual support
Works as a liaison with clergy of the patient’s choice
Bereavement Counselor
Bereavement counselors offer a variety of services to the patient’s family to help with understanding of grief
Hospice Volunteer
Hospice volunteers are kind and compassionate individuals who support patient’s and loved ones by providing respite care and friendly visit
