Care Giving Team

Medical Director

  • Certifies the attending physician’s prognosis

  • Leads the hospice team in developing a plan care

  • Provides consultation to the attending physician

Attending Physician

  • Certifies the patient has a terminal illness and remains the patient’s primary physician and works as a member of the hospice team

Skilled Nurses RN’s & LVN’s

  • Evaluate’ s patient’s condition during regular scheduled visits

  • The registered nurse coordinates the plan of care and teaches the family proper patient care techniques; the use of medication and management

Social Worker

  • Assess social needs of the patient’s, family

  • Provides psychological and Bereavement counseling

Chaplain

  • Provides spiritual support

  • Works as a liaison with clergy of the patient’s choice

Bereavement Counselor

  • Bereavement counselors offer a variety of services to the patient’s family to help with understanding of grief

Hospice Volunteer

  • Hospice volunteers are kind and compassionate individuals who support patient’s and loved ones by providing respite care and friendly visit
